Application Procedure

Applicants for Canadian Academy need to supply the following forms and documents, available as downloads for Grades 6 to 12 and BBP (Age 3) to Grade 5. Please note: your application will not be complete until the school has received all of the required items. See the Boarding Program Overview page for Boarding Application Forms.

BBP (Age 3) to Grade 5

  • Completed Application Form
  • Confidential Recommendation Form––sent directly by the teacher
    One confidential recommendation from a teacher is required for Elementary School students.
  • Signed Enrollment Agreement Form
  • School records for the past 3 years (including current school year) 
    • Report cards: Copies of the report cards of the past three years, including the current year are required. The final report for the current school year is required and can be emailed or posted when it becomes available if the applicant is accepted.
    • Applicants for BBP (Building Blocks Program – 3 year olds), KA (4 year olds), KB (5 year olds) who may not have report cards but attend a pre-school, nursery, or Daycare are asked to submit a recommendation from the class teacher
  • Standardized test results (ITBS, Stanford, etc.). 
  • Health form (to be completed by a physician and parent) 
    • Health forms must be completed before the child can begin school. The Tuberculin Skin Test for students applying to BBP – Grade 8 is required

  • Required immunization information
  • Photo copy of birth certificate or passport
  • Two passport size photos for each applicant. 
  • Application fee
    An application fee is required with each application. Please refer to the tuition and fees page for information about how to pay this fee. A receipt will be issued upon payment of the fee.
  • Additional reports, including from specialists if applicable
    • If the applicant has received or is receiving any supplementary school services or outside of school support, please include this report. These may include: learning resources support, speech and language therapy, English for Speakers of Other Languages (ESOL) support, counseling, behavior modification, etc. Others included are: psycho-educational evaluation, Individual Educational Plan (IEP), speech language assessment, etc.
    • Please include an exit report and recommendations for future learning if your child no longer is receiving these services.
    • Special Needs: Canadian Academy does not offer a full special education program; however, there is limited support for students with mild learning differences. Based on this, the ability to provide a program for a special needs student is contingent on the resources available and admission is determined on a case-by-case basis. If a child has undergone psycho-educational testing, copies of the reports are required. Students must be able to perform at grade level within a regular classroom.

Grades 6 to 12

  • Completed Application Form
  • Confidential Recommendation Forms––sent directly by the teacher
    Two confidential recommendations are required for Middle and High School students, one from an English teacher, and one from another subject teacher  
  • Signed Enrollment Agreement Form
  • Official Transcript (grades 10 to 12)
    • All high school students applying for grades 10 – 12 are required to include an official transcript from their current school. They should post or bring with them, an official transcript which includes the year just completed. If you have attended more than one high school, submit transcripts from all high schools.
  • English expository, persuasive or analytical paper sample marked by the English teacher (grades 9 to 12)
  • School records for the past 3 years (including current school year) 
    • Copies of the report cards of the past three years, including the current year are required. The final report for the current school year is required and can be emailed or posted when it becomes available if the applicant is accepted.
  • Standardized test results (ITBS, Stanford, PSAT, SAT, TOFEL, etc.). 
  • Health form (to be completed by a physician and parent) 
    • Health forms must be completed before the child can begin school. The Tuberculin Skin Test for students applying to BBP – Grade 8 and a chest x-ray for high school applicants (grades 9 – 12) is required
