Please download the appropriate forms found on the right-hand side of this page.

All new students entering CA are required to have  immunizations up to date  according to their age. Please download the appropriate information found on the right-hand side of this page. Copies of all vaccines received must be submitted to the health office for verification prior to the first day of school.

Students already attending CA who will be entering Grade 6 are required to have a physical examination completed by a physician and submitted to the health office prior to their first day of school.

Students already attending CA who will be entering Grade 9 are required to have a physical examination completed by a physician AND a Chest X-ray with results submitted to the health office prior to their first day of school.

The CA immunization policy was adopted using current information obtained from the World Health Organization (WHO), the Center for Disease Control and Prevention (CDC), the Japanese Department of Health and Welfare and various other worldwide health organizations.

Please keep in mind that many immunizations require multiple vaccines over an extended period of time. Compliance at the proper time for these vaccines is required.
